Purpose of Position
The Care Worker will provide in home, and community based support to clients who are frail aged or younger people with disabilities and their carers; as outlined in the clients Wellness Support Plan.
SDSA provides support to agency clients based on the Wellness Approach. This approach to support focuses on the client and setting achievable goals that staff can assist the client to reach. In some circumstances staff will be required to assist clients to learn or re-learn daily living skills that will enable them to remain living independently in their homes and the community.
The Care Worker will provide a range of different supports; these can be from the following service areas:
- Carer Support (respite for the primary carer),
- Social and Shopping Support,
- Home Care Activities.
Staff who are employed to provide carer support (respite) and social/shopping support to agency clients must have the correct qualifications and/or relevant experience.
Duties:
Section A – Carer Support (Respite)
The Carer Support Worker will provide direct care to clients in their own home or in the community, while providing respite to the primary care giver. This support will assist the primary care giver in their role, enabling them to continue to provide care to the client. This support is designed to provide the primary care giver time to rest or participate in the community.
The key responsibilities and performance standards may be modified from time to time, to ensure that the desired outcomes are in accordance with the clients Wellness Support Plan, which seeks to support the client to achieve the goals, identified and set by the client.
- Provide active and stimulating companionship and care for clients in their own home or the community.
- Prepare appropriate meals for clients when involved in all day respite (in consultation with the primary carer).
- Perform client care activities which would normally be provided by the primary carer, except where the client and/or agency staff member would be at risk; e.g. using hoists, assisting with toileting.
- Escort the client shopping or to medical/hospital appointments or elsewhere as agreed with the Client Services Manager. Communicate with clients and carers in a sensitive and caring manner.
- Assist clients in developing skills taking into consideration their disabilities/limitations.
- Carry out all duties in a flexible and efficient manner.
Section B – Social/Shopping Support
The Social Support Worker will provide support to clients of Southern Districts Support Association in their own home or in the community and to assist clients with tasks as outlined in the clients Wellness Support Plan.
The key responsibilities and performance standards may be modified from time to time, to ensure that the desired outcomes are in accordance with the clients Wellness Support Plan, which seeks to support the client to achieve goals, identified and set by the client.
- Provide social and emotional support to the client.
- Communicate with clients in a sensitive and caring manner.
- Provide companionship and support to client.
- Provide transport and escorting for client.
- Provide active and stimulating companionship and care for clients in their own home or the community.
- Escort the client shopping or to medical/hospital appointments or elsewhere as agreed with the relevant manager. Communicate with clients and carers in a sensitive and caring manner.
- Assist clients in developing skills taking into consideration their disabilities/ limitations.
- Assist in maintaining a friendly and safe environment for the client while out in the community.
- Carry out all duties in a flexible and efficient manner.
Section C – Home Care Activities
The Home Care Worker will provide assistance to SDSA clients with home care tasks as outlined in the clients Wellness Support Plan.
The key responsibilities and performance standards may be modified from time to time the ensure that the desired outcomes are in accordance with the clients Wellness Support Plan, which seeks to achieve identified goals set by the client.
House cleaning activities as agreed with the manager and written into the client’s Wellness Support Plan and which may include:
- Typical home care activities staff may assist the client with are –
- washing and vacuuming floors,
- cleaning bathrooms,
- laundry and ironing, and much more.
- Communicate with clients and carers in a sensitive and caring manner.
- Assist clients in developing skills taking into consideration their disabilities/limitations.
- Provide companionship and support to clients.
- Communication:
- Carry out instructions and follow advice from the Home Services Manager in relation to each clients support.
- Complete ‘Feedback Report’ form and hand to Home Services Manager or into SDSA office. The report should contain information regarding concerns or changes to clients health and/or circumstances.
- Maintain confidentiality of client, family/carer and SDSA information at all times.
- Attend scheduled meetings as organized by the Home Services Manager.
- Training:
- Attend orientation and induction training within 1 month of commencement.
- Attend client-specific training as required.
- Attend compulsory training to maintain competency as required for this position.
- Policies & Procedures:
- To work within the guidelines of SDSA Staff Orientation Manual, including agencies policies and procedures.
- Occupational Safety & Health:
- Attended OSH Induction within 1 month of commencement and annual refresher training offered by SDSA.
- Be familiar with emergency procedures to ensure client and staff/volunteers safety.
- Staff are required to report all accidents, incidents and hazards to SDSA and complete documentation.
- Ensure Occupational Safety and Health standards are maintained on sites and that any OSH concerns are reported.
Administrative:
- Ensure time sheets are completed accurately, signed and submitted to the office by 4pm Wednesday.
- Accurately record expenses incurred and receipts attached to time sheet.
- Ensure personal contact details are up to date.
- Read and sign the Staff Orientation Manual annually.
- Attend annual performance appraisal.
- Carry out all duties in a flexible and efficient manner.
- Monitor client wellbeing and provide feedback to relevant Manager.
- To collect and receipt client fees as requested.
Skills and Experience Required
Essential Criteria
- Able to communicate with clients effectively and respectfully.
- A strong commitment to working with people with disabilities and frail aged persons.
- Participate in training programs, staff and volunteer social functions and staff appraisal.
- Efficiency and care with performing household duties.
- If providing Respite or Social Support must hold Certificate III in Community Care or similar tertiary training. SDSA will also consider applicants with experience and who have attended training courses.
- Ability to work as part of a team and/or independently.
- A current WA motor vehicle licence is required - ‘C’ class and own vehicle.
Experience in working with people with disabilities or the frail aged.
- Holder of a current First Aid Certificate (or willing to obtain within six months).
- A National Police Clearance is required before commencing work with SDSA.
- Willingness to embrace and abide by SDSA ethos, policies and practices.
- Ability to respect the Christian ethos of the Association.
- Apply Occupational Safety and Health standards.
Desirable:
- Experience in working with frail aged and people with disabilities.
- Experience cleaning either commercially or domestically.
- Basic knowledge of disability.
- Basic knowledge of mental health.
- Basic knowledge of dementia.
